Welcome to Gratus Community Care, where our mission is to deliver equitable, culturally competent, and member-centered services. We proudly serve the community under New York State’s 1115 Medicaid Waiver Social Care Network (SCN). Beyond health navigation, we support immigrant families through our Pathway to Citizenship initiative, offering access to education, resources, and culturally responsive guidance as individuals pursue stability, empowerment, and full participation in their communities.

SCN Program (Social Care Network)

The SCN Program focuses on providing holistic support to those who need it most by connecting individuals with Medicaid-funded benefits and services. We offer the following services:

Screening & Enrollment

  • Initial Assessment: We complete an assessment to determine which Medicaid-funded benefits, services, or programs you may qualify for under the SCN.
  • Identify Needs: We identify immediate needs related to housing, nutrition, transportation, safety, and social supports.

Navigation & Care Management

  • Comprehensive Assessment: We determine if you need Level 1 (L1) or Level 2 (L2) care management.
  • Coordination: We coordinate services, provide follow-up, advocacy, and linkage to community supports.

L1 - Basic referral services.
L2 - Enhanced Health-Related Social Needs (HRSN) care management for those with higher vulnerability.

Eligibility for L2 care may include:

  • Homelessness or risk of homelessness
  • Food insecurity
  • Utility insecurity (e.g., shut-off notice, arrears)
  • Transportation insecurity
  • Housing quality or safety issues
  • Recent hospitalization or high medical utilization
  • Chronic health conditions (physical/behavioral)
  • Caregiver stress or inability to meet daily support needs
  • Functional limitations with ADLs/IADLs
  • Risk of institutionalization
  • Domestic violence or unsafe living conditions
  • Financial instability affects health

Housing Supports

  • Home accessibility modifications & safety repairs
  • Rent assistance or temporary housing placement
  • Utility activation or arrears support
  • Housing Transition Services, including:
    • Pre-tenancy services
    • Community Transitional Support (CTS)
    • Housing navigation
    • CTS broker fee assistance

Nutrition Counseling & Education

  • Provided by Certified or Registered Dietitian Nutritionists
  • Individual nutrition assessments
  • Healthy meal planning, education, and cooking-supply support
  • Guidance for managing conditions like diabetes, high blood pressure, obesity, allergies, and age-related needs

Pathway to Citizenship (Community Education Program)

At Gratus Community Care, we proudly support immigrant families and long-term residents through our Pathway to Citizenship community education program. Our goal is to make the naturalization process less overwhelming by offering welcoming, bilingual guidance, basic information, and connections to trusted community resources.
